For Active-Duty Preventive Health Assessments and PRE and POST Deployment Assessments please visit the new Preventive Health Assessment Website at:  https://eha.health.mil/eha/

To access the website:

1. Log- in using your CAC card.
2. Select "My PHA" or "Electronic Deployment Health Assessment" and complete the questionnaire.
3. After completing the questionnaire, you may walk-in or call the Readiness Clinic at Branch Health Clinic Apra Harbor at (671) 339-3226.
